• 3+1 Internship Cover Form (photo to be attached)

• Internship Diary documenting the weekly tasks performed

• Attendance Status (to be written in one sentence for each task completed)

• Grade Report Document (login to firma.bilecik.edu.tr using the workplace tax number and password, enter grades scoring out of 100, then print the document and obtain signatures and stamps/seals)

All sections requiring signatures and stamps must be properly completed, and the documents should be submitted to the advisor in a transparent folder.
